Your Docs Lie.

A stale doc used to be a minor annoyance — a human reader would notice the API had moved on and adjust. An AI agent doesn't do that. It reads the doc as ground truth and acts on it with full confidence, at machine speed, with no hesitation to flag the mismatch. The fix isn't writing more comprehensive documentation; it's keeping the docs you have actually current.
